We have developed an innovative therapeutic approach, "MyPLAN" which combines recommended best-evidence therapies and tailoring of these to the individual and their valued activities using AI and Technological Innovations.
We will deliver the MyPLAN approach to stroke survivors living in the community to improve access and outcomes, and guide the clinician and individual with stroke in ongoing recovery.
 
                            Stroke survivors will work with a TAILOR and CONNECT Therapist to tailor their MyPLAN. This will be developed taking into account the individuals current abilities and their identified functional tasks they wish to achieve. They will then participate in bursts of therapy in their own home via telehealth (or at a clinic if they prefer) following their MyPLAN.
 
                            Individuals will wear sensors and use an experience sampling app that will help profile stroke survivor’s needs in real-time, with AI-assisted computer vision embedded in assessment and therapy sessions providing interactive feedback on movement quality and valued activity performance, guiding the progression of therapy sessions and opportunities for independent practise outside of therapy.
 
                            Therapists are able to conduct sessions with the stroke survivor in their own home via telehealth. Stroke survivors are provided with all the necessary therapy equipment that they will require for the duration of their therapy program. Having the equipment at home provides opportunities for the individuals to independently practise the various therapy components and task practise on a daily basis as set by the Therapist, who will then be able to progress the program as gains are made.
 
                            Your therapist will work closely as your coach. Coaching sessions will focus on improving skills and performance in your chosen activities. Therapy coaches will help you improve how you discover and learn in your chosen activity to solve challenges that you may encounter. They will then tailor your therapy sessions and prescribe a tailored therapy exercise program for you to perform outside of therapy sessions.
 
                            Your therapy coach will provide you with all the necessary equipment needed for you to be able to practise activities learnt in therapy sessions, and prescribe a recommended program for you to do this outside of therapy. This will allow more opportunities for progression at each therapy session.
